Qualifications:
· Degree/Education (5.2.4): The Neurologist must have four years of medical school resulting in a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree who is board eligible or board certified in the specialty of either neurology or physical medicine.
· Certifications in addition to Basic Life Support (5.2.3): Possess and maintain at all times, at Contractor expense, current certification in Basic Life Support (BLS). BLS is not the same as cardiopulmonary resuscitation (CPR) training. The American Heart Association Basic Life Support for Healthcare Providers is the only program accepted.
· Experience: Shall have a minimum of two year experience within the last two years working as a Neurologist Physician. Possess fellowship training as a nerve muscular specialist or in electro diagnostics equivalent to one full year.
· Board Certification (5.2.6.4.4): Board Certified by the American Board of Psychiatry and Neurology.
· Licensure/Registration: Contract provider shall possess a current, unrestricted license to practice medicine and a current, unrestricted Drug Enforcement Administration (DEA) registration/Certificate. Meet all licensing/certification requirements to perform as a Neurologist. Physician must be currently licensed in one of the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands and be able to obtain clinical privileges to practice medicine with the MTF Credentials Committee.
