Seeking a BE/BC Geriatrician or BC IM or FP Physician with experience and a passion for working with the Geriatrics patient population to join Geriatrics team consisting of one full-time Geriatrician three full-time Nurse Practitioners and part-time Palliative Care Internist. You will have the opportunity to round on nursing homes assisted living facilities and adult family homes as well as make home visits on home bound patients. You may also maintain office practice if desired. Office and nursing home call is shared; provide consults to Transitional Care Unit and Palliative Care. Your practice may grow to include outreach services to nearby communities. You must enjoy working within a multidisciplinary team.
