Reimbursement Information:
· The rate of reimbursement for collaborators is dependent upon number of Nurse Practitioners and State assigned.
Requirements:
· Licensed in the state of practice (MD/DO)
· Current License is Active in the state of practice
· Primary Specialties in Geriatrics, Family Practice or Internal Medicine (GP, FP, IM, Gero) preferred.
· Board Eligible or Board Certified
· Currently not enrolled in the Medicare Opt-Out program
