We are seeking a BC/BE Family Medicine/Internal Medicine physician to serve as a community Care Discharge Coordinator. The focus is to ensure a seamless transition of care and patient re-entry or initial entry to the Clinic. Main Roles/ Duties: Physician will serve as the point of contact for the community hospital inpatient discharges Review/reconcile medications and any durable medical equipment needs Arrange home health/community care referrals prior to discharge Coordinate post discharge appointments Review pertinent data from discharging facility to succinctly summarize for PCP to review and co-sign Address patients urgent needs/concerns between time of discharge and follow-up with the PCP Opportunity for long term hospitalist position at the Virtual Inpatient Unit and/or the new state-of-the art Medical Center or permanent primary care position.
