Hospitalist Place of Performance: Crownpoint, NM A-Team Solutions is seeking a Hospitalist for a small rural hospital with an Emergency Room, Urgent Care Clinic, and numerous ambulatory care clinics. The Inpatient Unit has an average of 5-8 patients daily with a typical maximum of 12 patients. There is no Intensive Care Unit. The ER has 5 beds. Ground and air transport is available for patient transfers to higher level care facilities in the region (located 1-2 hours away by ground). STATEMENT OF WORK 1. Provide Hospitalist services on-site at the Facility's Inpatient and Observation units with some visits conducted in the emergency room or clinic if the patient is to be admitted. This work includes the assessment, diagnosis, treatment, care coordination, consultation, referral, transfer and/or discharge of low to moderate risk patients, and the stabilization and transport of more critically ill patients. The Hospitalist shall conduct histories and physical examinations, and prepare discharge summaries for each patient. 2. The Hospitalist contractor will document all care on the electronic health record (EHR), complete all billing and encounter forms, complete appropriate referral forms, and contact Purchased/Referred Care Services as appropriate in order to refer patients to specialty services not available at the Facility. 3. The Hospitalist contractor may provide consultation services to other providers. The Hospitalist shall be responsible to transfer care to other inpatient or on call providers including information about the patient, clinical findings, discharge plans, and other care coordination details. 4. Provide Urgent Care services on an occasional basis when the inpatient service is not busy. The contracted family medicine doctor will be assigned to Urgent Care, and satellite clinics. These shifts will be 10 hour shifts, within the 8 am-6 pm time frame. The provider will be responsible for covering all of the patients that present to urgent care during this time period. This could include up to 100 patients a shift. 5. The contractor will work the hours specified by the Medical Staff Schedule. In emergency cases, the contractor may be required to work additional hours, but will be relieved as soon as possible. The extra hours will be charged at the same rate, and not exceed two hours at any one time. 6. The contractor shall attend medical staff morning report at 7:30 a.m. daily during assigned work days, either to get updates on patients in the ER at the beginning of a shift or at the end of a shift to report out on admissions, transfers, and other cases. 7. Provide Emergency Room services on a rare basis such as when a medical disaster occurs, when the ER is overloaded, when a Code Blue occurs, or other unusual and rare instance. Emergency Room services provided by the contractor in these instances shall focus on low to moderate risk patients. Emergency Room services provided by the contractor shall exclude trauma and OB. 8. The Hospitalist shall abide by the Facility medical staff bylaws, medical staff rules and regulations, hospital policies and procedures, and Indian Health Service policies and procedures. Qualifications The Contractor shall provide to the Clinical Director of the following documentation: 1. A current, active license from a state medical board. 2. A completed clinical privileges application. 3. Proof of Board Certification in Family Medicine or combined Internal Medicine Pediatrics board certification. 4. Letters of Recommendation from current or previous supervisors (2). 5. Proof of BLS, ACLS, and PALS certification. 6. Proof of current immunizations. 7. A current driver's license.
