Education and Experience for the Physician:
Doctor of medicine or osteopathy required.
Current Florida license required.
1 year Geriatric care experience required
Board Certified in Family Medicine or Internal Medicine required
Geriatric experience preferred
Valid Florida Driver’s license with current automobile insurance and reliable transportation
Must clear level 2 background check
