AS SOON AS PROVIDER CAN BE CREDENTIALED – 6 MONTH ASSIGNMENT minimum

Will the Doctor be seeing clinic patients during the day – Yes in the AM / IP in afternoon
If so, do you know which days? Tuesday / Wednesday / Thursday
Are ERCPs required? Preferred but not necessary
Is the clinic located in the hospital or offsite? Offsite / Across the street
Provider will be required to provide supervision of CRNA at OP Clinic
Approx 12 patients + 2-3 hospital consults
Hospital EMR – Cerner / Clinic – GE Centricity

Required procedures (ex: EUS, ERCP)? No but preferred
Board certification requirements? YES – Must be BC or BE
Standard shift time (mainly need to know what the outpatient hours would be in the clinic)? 7:00am – 12:00Noon
Beeper call time (night/weekend/both)? Both
How often do they get called back? Average less than 1 time per night
# of patients seen per day (inpatient vs outpatient and how many are new consults)? 10-15 Mixture / 5 new consults
How many procedures per day on average? 10-12
Types of acuity they will be seeing? At time - severe bleeder
24hr Anesthesia
