Medical Director (FP, IM) – Post Acute Care opportunity close to border of NY and VT

 

Seeking an experienced Board-Certified Family Medicine or Internal Medicine physician to join a practice providing primary care to adults in Long Term Care facilities.   

 

Full-time position – 3.5 – 5 days per week

Job Duties:

  • Primary care rounding in SNF and Facility Medical Directorship.
  • Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care.
  • Collaborate with clinicians, facility leaders and others regarding care and services.
  • Build relationships with your patients, patients’ family members, and facility staff to facilitate effective treatment delivery.
  • Provide oversight, guidance, and mentorship to our Nurse Practitioners.
  • Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care.
  • Represent company on appropriate external levels engaging and establishing/maintaining relationships within the post-acute care community.
  • Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues.

Will not accept visa candidates. Candidates must have at least 1 year post residency or fellowship experience.

 

Competitive Compensation, with an estimated salary range of $170,000/annually to $250,000/annually with the opportunity to earn incentives.
