This Patient- Centered primary care practice encourages patients to view the center as their medical home; patients are uninsured or on Medicaid or Medicare; Practice offers mental health counseling and pediatric dental services. Practice also has support services available to patients including the drug assistance program the We Care program for specialty referral and translation. 60% of clients suffer from chronic conditions including diabetes asthma and hypertension. Average number of patients per day is 24. NO CALL schedule (ARNPs take call). No critical care chronic care management only.
