REQUIREMENTS:

1. Shall have a current license to the practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia.
2. Must be Board Certified or Board Eligible in Internal Medicine or Family Practice.
3. Shall be technically proficient in the skills necessary to fulfill the government’s requirements, including the ability to speak, understand, read, write, and type English fluently.
4. Shall provide copy of current Drug Enforcement Agency (DEA) certificate.
