• Monday - Friday schedule, 8 am - 5 pm with minimal triage call
• Shared call among all providers; RN call center handles routine calls
• Only urgent matters escalate to the physician, supporting great work-life balance
• Manageable patient census of 10 - 12 patients
• Value-based care model with focus on quality outcomes
• Experience with eClinicalWorks and HEDIS highly valued
• DEA license required
• Board-certified or board-eligible geriatricians encouraged to apply
