• 8 am - 5 pm schedule with minimal shared call coverage
• Focus on complex geriatric patients ages 65+
• Value-based care experience with HEDIS measures preferred
• Work with 2 strong APPs in collaborative team environment
• eClinicalWorks EMR system and HCC coding experience valued
• Board-certified or board-eligible in geriatric medicine
• No residents, direct patient care focus
• Spanish fluency preferred but not required
