Locum Tenens Hospitalist: Family Medicine Job in Bath, New York
About the Facility: Bath VAMC
Bath VA Medical Center, part of VA Health Care Upstate New York is in the
Southern Tier of New York State. The Medical Center serves Veterans in
Allegany, Chemung, Schuyler, Steuben, and Yates counties in NY, as well as
Veterans in Tioga and Potter counties in Pennsylvania.

Nocturnist/Hospitalist - RESPONSIBILITIES (listed are
included but not limited to)
- This
is a night shift from 4 pm - 8 am with the requirement of rotated coverage
for weekends/holidays, previously agreed upon mutually beforehand.
- Assessment
of in-house patients varies in workload but is the primary responsibility
of the MOD on duty.
- MOD
tourts on evenings, weekends, and holidays to include patient care
rounding in the CLC, RRTP, and Acute Psych, as necessary and included in
this coverage.
- Must
follow all established medication policies and procedures.
- Provide
discharge education and follow-up instructions coordinated with the next
care setting for all emergency department patients.
- Must
intubate and complete OOORAM when orienting.
- Shall
complete the appropriate QM/PI documentation related to all procedures,
complications, and outcomes of examinations.
- Must
adhere to all established patient safety and infection control standards
of care.
- Obligated
to report all events related to patient injury, medication errors, and
other breaches of patient safety to the COR VA Safety Policy. Incidents
should be reported within 24 hours, and the Patient Safety Report should
be submitted to the COR, following up as required or requested.
- Must
make every effort to prevent medication errors.
- The
Hospitalist should take measures to prevent patient injury caused by acts
of commission or omission in the delivery of care.
- The
Primary Care Physician performs comprehensive medical history evaluations
and physical examinations on patients ranging from adult to geriatric
populations.
- Diagnose
medical conditions and order appropriate laboratory tests, imaging studies
(e.g., X-rays), and other diagnostic procedures as needed.
- Collaborate
with other agencies or health facilities on behalf of patients and their
families to arrange admissions to hospitals, rehabilitation facilities,
and skilled nursing facilities.
- Organize
emergency ground and/or air medical transport as required.
- Analyze
clinical data, develop therapeutic programs, and communicate information
effectively to patients and their families.
- Maintain
accurate and timely Electronic Health Records (EHR) with detailed clinical
documentation, including diagnosis codes. Complete all EHR notes within 24
hours of patient encounters.
- Prescribe
medications and treatment therapies in coordination with nurses,
pharmacists, and other healthcare personnel.
Nocturnist - REQUIREMENTS
- Graduate
from an accredited School of Medicine with an MD or DO degree.
- Board
Certified or Board Eligible in Internal Medicine or Family Medicine is
required.
- Must
possess a current, full, and unrestricted license to practice medicine in
any U.S. State, Territory, Commonwealth, or the District of Columbia.
- Must
hold a current Basic Life Support (BLS) and/or Advanced Cardiac Life
Support (ACLS) certification recognized by the American Heart Association.
