Organization: Oak Street Health Title: Primary Care Physician Oak Street Health is an innovative network of 15 neighborhood primary care centers in the Midwest focused on providing high quality care to older adults. Our physicians practice with a full team behind them; each care team is made up of: • Physician • Registered Nurse • Medical Assistant • Clinical Informatics Specialist (i.e., a “scribe” to manage documentation) • Care Manager • Nurse Practitioner Our facilities are all new, designed around this model of comprehensive, personalized primary care.
