Two offices: Starke (1 mile from hospital), office is actually a renovated wing of the original community hospital, with over 2,600 square feet and 4 exam rooms. Keystone Heights (15 miles from hospital - towards Jacksonville and Clay County) Practice also employs two Nurse Practitioners. Option for partnership after 18 months.
